  Prior to scheduling you for an appointement you will need to fill out the Health History Form and mail it to our office @ Balance Your Chemistry 2828 S. Arlington Rd Akron OH, 44312. We will also need your records from your physician. The authorization of release of records form is the Health History Form. Once all this information is recieved at our office. We will contact you to make and appointment.
